The above forms can be completed electronically, but must be printed, then signed and either faxed to 03 9813 4215 or posted to Level 1, 123 Camberwell Road Hawthorn East VIC 3123.

Please remember to include proof of purchase such as a copy of an invoice or a plumbers or electrician’s certificate of compliance and please include your email address to receive your payment remittance advice.
